The Certificate in Field Epidemiology is a 12-credit hour program of study. The curriculum addresses the core functions of:

  • outbreak investigation
  • surveillance systems and methods
  • infectious disease epidemiology
  • field epidemiology methods

All courses have been specially designed and developed for delivery via the Internet. Students will be able to register, receive materials, interact with faculty, order books, and successfully complete this program without traveling to campus. A connection to the Internet is all that is required.

Your application requires a written statement to be uploaded within your application. Your written statement is a critical component of your application for admission and can sometimes be the determining factor in your admittance. Therefore, your statement should reflect your leadership or management experience, your interest and experience in health care or public health, and your professional goals as well as familiarity with the program and faculty at UNC-Chapel Hill.  Detailed instructions are below.

Upload a document that answers each of the following questions in numeric order (the entire document should be no more than 2 pages).

  1. State your purpose in applying for the Certificate in Field Epidemiology program.
  2. Explain how the certificate will be of value to you.
  3. Describe any experience you have had with online/distance education.
  4. Ten-twelve hours of student work per course, per week are required for successful course completion. Describe how you will schedule that time.
  5. If you will be working full-time, explain what mechanisms you have or can put in place to ensure that you will be able to handle the demands of both work and school. Specifically, detail the support your supervisor has offered and whether release from work/school is available.
  6. Describe what contributions you could make to this certificate program.
  7. Please list all college-level math and/or statistics courses you have taken, and the grade earned for each – (honors courses taken in high school can be included).
