Letters of Recommendation 3 2 from former professors , and 1 from someone who may attest to your character and potential to succeed in the DH program.
International Applicants Only TOEFL or IELTS score (no more than two years old)

Other Requirements

In addition to the University’s required application materials, applicants to the dental hygiene program must upload the Dental Hygiene Supplement and the Dental Hygiene Shadowing Documentation form. These forms may be found by selecting Downloadable Forms from the menu on the left side of your online application.

The pertinent online application section, relating to each requirement, is indicated by [brackets].

The Dental Hygiene Supplement has three parts.

  1. Discuss information about your interest and exploration of dental hygiene as a profession.
  2. Provide a statement of your professional goals.
  3. Discuss additional factors you would like considered in regard to your application. [Supplemental Documents #1 Upload]

The Dental Hygiene Shadowing Form

Complete a minimum of 20 hours of dental hygiene shadowing.  Consider visiting private practice dental offices, community dental clinics, or volunteering at dental health events such as MOM (Missions of Mercy) clinics.  Use the shadowing form to record the location, date, number of hours, and supervisors signature for each experience. Final Note:

For an application to be considered, all of the pre-requisites must be completed by the time dental hygiene classes start in the fall. However, it is strongly recommended the science courses be completed, or in process, no later than the spring semester you apply.
